*Medical Liability Release & Acknowledgment of Risk: I hereby certify that the above named enrollee is in good health and free from any communicable disease. In case of medical emergency (taking place during the course of the event/tour) I hereby give permission to representatives of Blue Amber Travel and/or its affiliates/or organizers of the event to secure proper treatment for enrollee. I also certify that I am responsible for my insurance company or myself will cover all medical treatment costs. I acknowledge that some of the activities provided by Blue Amber Travel, e.g. skiing, rafting, canoeing, swimming, horseback riding, hiking, biking,boating,sightseeing and others, may entail known and unanticipated risks which could result in physical or emotional injury, paralysis, death, or damage to myself or participant, and/or to property, equipment or to third parties. I understand that such certain risk cannot be eliminated without jeopardizing the essential qualities of the activity. I expressly agree and promise to accept and assume all of the risks existing in this activity and certify that my participation in this activity is voluntary and I elect to participate in the activities despite the risks involved. I also hereby voluntarily release, forever discharge and agree to indemnify and hold harmless Blue Amber Travel from any and all claims, demands, or causes of action that may be in any way related to or result from my participation in any activity. I hereby release Blue Amber Travel, it s representatives, and owners of any facility used by Blue Amber Travel from any and all liability.

Enrollee Responsibility Notice: Blue Amber Travel and/or it s agents act only as an agent in making arrangements for hotels, sightseeing tours and transportation or any other service in connection with the event. Blue Amber Travel and/or its agent or suppliers shall not be liable for injury, death, damage, loss, accident, delay or irregularity, liability or expense to person or property due to act or default of any hotel/motel,or other accommodation property,airline, carrier or other company or person providing or rendering services included in the event, or any and all acts of a third party. Further, the sponsoring organization and/or its agents accept no responsibility for any sickness, labor disputes, defect in any vehicle of transportation, or for any misadventure or casualty, or any other cases beyond their control. Blue Amber Travel reserves the right to cancel, change, or substitute any service and to decline to accept or retain any event member at any time for any reason including insufficient number of participants. Your registration provides Blue Amber Travel the authorization to use photos and videos of you and your registered participants for promotional purposes. This document is valid for each event organized by Blue Amber Travel, updating all necessary information about participant is responsibility of enrollee. We strongly recommend that all travelers purchase any TRAVEL INSURANCE to protect against the risk of medical expenses, death, travel delays, trip cancellation, any damage, loss of baggage or personal belongings, etc.
